This will be the  second clinic for those that received the first dose (on 12/14/21) on Tuesday, January 4, 2022 from 3:30-6:00 p.m.

You will need to fill out a form to give consent as well as be present when your student is vaccinated.  We will NOT be vaccinating any students without parent/guardian consent AND parent/guardian attendance at the clinic.

To sign your student ages 5-17 up for the clinic please fill out a form and email it to [email protected] .  The Vaccination Clinic will be open to all students, regardless if they are AKFCS students, as long as they are registered and accompanied by an adult.

The clinic will  have booster shots available for adults and those 18+ who need it. We will also offer initial vaccinations for adults (anyone 18+).  Anyone seeking a vaccination or booster shot, must register by emailing your name, contact information and date of birth to [email protected]. This will ensure we have enough doses on hand to accommodate those who will attend.
