Enrollment and Health Records

The AKF Nursing Department is committed to ensuring student safety. When enrolling a students at Abby Kelley, the following health and medical information is required for all students.

• Immunizations

  Proof of all required immunizations, including the dates of each immunization
• Lead Test
  A lead test is required for all students entering Kindergarten
• Vision Screening
  A vision screening is required for all students entering Kindergarten

Does your child have allergies and/or require the use of medication during the school day? If so, our Nursing Department also needs the following:

• A signed consent by the parent or guardian to give the medication in school
• Signed medication order and action plan if indicated by the school nurse. The written medication order form
  should be taken to your child’s licensed prescriber (your child’s physician, nurse practitioner, etc.) for completion and
  returned to the school nurse.

• Families must make arrangements to drop off medication to the school nurse, as student’s are not permitted to transported medication per state law and school policy.

Massachusetts state law requires all public school students to receive specific vaccinations against diseases.

If you need help obtaining medical records for your student, you may visit a local clinic that provides free physical exams and school immunizations through the Worcester Free Clinic Coalition or https://www.worcesterfreecare.org/ .
